Below are the required forms needed to enroll your child. After initial application is returned you need to send over the scholarship award number, if applicable. The additional forms are also required to be filled out and returned in order for your child to be fully enrolled. After completion please send to onboarding@achieveabilitytherapy.com or return in person to our admin office at 18284 US-41 Lutz, FL 33549.
